An Engagement Plan captures significant details, events, and memories from a person’s life in a personalized narrative. It helps caregivers, family members, and healthcare professionals understand and connect with an individual on a deeper level. By creating an Engagement Plan, we improve the quality of care, making it more person-centered. Personal information enhances communication, providing comfort and a sense of identity for individuals with dementia.
At Vitalia North Olmsted, we gather this information as soon as possible, before a resident moves into our Memory Care Neighborhood. As dementia progresses, it often becomes more difficult for residents to communicate their wants, needs, and wishes. These plans help us meet their desires throughout their dementia journey.
The Engagement Plan is an in-depth profile that provides comprehensive information about your loved one, including their care needs, interests, and history. It includes their favorite family recipe and the names and occupations of their parents, creating a complete picture of who they are.
For someone living with dementia, an Engagement Plan is vital. It serves as an essential tool for the care team. If your loved one experiences sundowning and asks about their parents or children, the plan helps the team understand who they are referring to. This knowledge builds trust and enables the team to de-escalate difficult situations. Additionally, it guides the team on how to best engage your loved one, whether they prefer group activities, one-on-one interactions, bingo, or reminiscing.
To prepare for the future care of your loved one, start by asking them questions. Creating a legacy book together can be a great way to start conversations. Gathering this information now, while your loved one can still participate and provide input, will be immensely helpful in the future. If multiple family members can contribute, reach out to them. The more stories and information we have about your loved one, the better we can care for them.

Include what makes you happy, sad, and fearful. Mention any special things that bring you joy and comfort.
